Reliable Nanny for two school aged kids

We live in Central Park, CO and have two active boys (ages 11 and 14), a sweet standard poodle, and active work schedules.
We are looking for help 5 days a week in the afternoons and early evenings to help get kids picked up form school and drive them to after school practices/activities. A typical week is 20-30 hours.
We live in NE Denver and Charlie (11yrs) gets out of school at 3:10pm and Coleman (14 yrs) takes carpool home and needs rides to practices/activities between 5pm-6pm. Light housekeeping, snack prep, and a dog walk required daily.
