I am a certified End of Life Care Companion and I have two years of experience as a Personal Care Aide. during this time I was overseeing the primary care and assisting with all daily activities for one patient. My responsibilities included -

Personal Aide — provide 24 hour, in-home care including personal hygiene, lift 50+ pounds to transfer from bed to wheelchair, physical fitness, shopping, cooking, eating, accompany to appointments, mental stimulation, soothing presence
Medication — organize + distribute medicine, maintain communication with care team, adjust medications as needed
Liaison between primary care providers and patient
Ensure a safe, clean, and sanitized home environment for patient

I live in Manhattan (Morningside Heights).
