• Needs light help with cognitive activities, including cooking, remembering tasks or dates, phone communication
• Needs significant help with cognitive activities, including running errands
• Needs significant help with mobility activities, including mobility, lifting objects, shopping
• Has the following medical issues: heart problems, emphysema/COPD, diabetes, kidney problems, liver problems, high blood pressure/hypertension
• Needs medical assistance at times for taking medication, administering insulin, taking vitals
Care Recipient
- Male
- Very receptive to outside help
- Age range: 75 to 84 years old
