Homebound Mother requires grocery and pharmacy shopping

My homebound mother-in-law, 78 years old, is homebound due to severe anxiety. Currently, my wife picks up her prescriptions and orders, picks up, and delivers her mom's groceries.
My mother-in-law is weak physically. She experiences dizziness and has fallen and injured herself on more than one occasion.
My mother-in-law doesn't have a computer or mobile phone, so taking responsibility for self-care re: ordering groceries and prescription refills is not an option and is currently handled by my wife.
Because my wife and I are moving out of the country, we need the following:
* a reliable, professional, and patient caregiver who can stop by my mother-in-law's home twice per week
* check-in with my mother-in-law by telephone on other days per week when not visiting in person
* Help order groceries, pick-up groceries, and deliver groceries
* Pick up prescription medications that cannot be delivered by pharmacy.
